In order for the fundraiser’s campaign to be properly credited for a mailed-in donation, each check must be accompanied by this completed form. Please fill out the form below entirely, making sure to write the fundraiser’s name in the “‘Donation to Sponsor” line. Please write his or her name in the “memo section” of the check as well. Make the check payable to Travis Manion Foundation and send it to:

This donation will be entered into the online donation page when the check and form are received by the Travis Manion Foundation. It will appear as an “offline donation”.
